Help replicating optin form

Hi, 

Hoping someone can help point me in the right direction to replicate this optin form by Bob Ross (product was TrapFunnel). Unfortunately he says its just a basic form with little (read none) explanation of what he used to create it.

I've attached an image to better show how it works;

From what I can tell from the photo- the first step is to enter your address. Once done the second part of the optin requests your name and email, send it to me and done.

I've used (and use) a few form creators that use conditional logic but all are limited without a boat load of custom work. I simply would like this basic page with the double optin.

Any and all input is appreciated.

If you don't want to read it entirely then read this, in 9 out of 10 cases pop-ups have brought more subscribers, had no negative impact on bounce rates or engagement.

It seems that pop-ups work but you need to time them correctly. The offer needs to match the content.

Pop-ups aren't irritating if you don't slap them across visitor's faces as soon as they land on your site.
 
 
 
which exit popup do you guys use?

Depending on your niche - users have been trained over the years to view anything that pops up as a virus warning or something and just click them closed.
 
 
 
Don't forget - its not just about using an opt-in popup to increase signups. It's also about what you are offering in the pop-up. To really increase signups in pop-ups, always use a great free incentive to entice visitors to give their email - a free ebook, a free consultation, a first time purchase coupon, etc. Don't just say 'sign up to get updates' or 'join our newsletter' - those aren't very enticing.
